Lumeon has been working with a leading health plan insurer and wellness provider to improve efficiency and transform their patient experience.

Using Lumeon’s CPM platform has enabled the client to streamline the HRA process, from automated member reminders and convenient online booking, to generating personalized HRA 360 reports for each individual. Lumeon experts also worked closely with the client to roll out a new digital lifestyle coaching service, supported by virtual engagement, automation and goal tracking.

To do this required the standardization of care delivery processes across 14 separate IT systems, plus a variety of manual care coordination methods.
