It first screens patients by sending them a link to a risk assessment survey, which determines their risk level by baselining their comorbidities and symptoms. Low-risk patients are offered guidance including self-care advice or testing information, while high-risk patients are enrolled onto the COVID-19 Remote Home Monitoring program. Patients enrolled in the program are automatically surveyed twice daily via SMS about their breathing difficulty and are automatically connected to clinicians if responses indicate deterioration.
